Each family referred to our Nashville center should receive a welcome packet in the mail which includes any necessary form as well as instructions to complete the next steps in the registration process. Please complete the packet so that we can prepare for your child’s evaluation. *If you would like assistance in completing the forms, one of our team members would be happy to help you. Please call us at 615.730.8095 to schedule an appointment with a team member. Completed forms may be returned via mail or fax.

Help! I lost my new patient packet. What should I do?

Feel free to call our office at 615.730.8095 to have another packet mailed to you. You may also  download the new patient forms and print prior to your visit. Forms for  school personnel are also available to download. 

What should I bring to our first appointment?

Please plan to bring the following:

  • Screening evaluations (hearing, vision), Educational/school testing, or other relevant testing results
  • Individualized Education Plan (IEP)/Individualized Family Service Plan (IFSP), psychological, neurological, other evaluation/testing notes, and therapy reports (Speech Therapy, Occupational Therapy, Physical Therapy)
  • List of all medications (prescriptions, over the counter and supplements) 
  • Insurance Card(s) 
  • Copay/Co-insurance or deductible (Our office policy is to take these payments at the time of service.)

How long will our initial appointment last?

Your first appointment may be up to 2 hours in duration. This gives our team the opportunity to understand your concerns, your child’s developmental history, and evaluate your child. We also ask that you arrive 15 minutes prior to your appointment time to ensure the check-in process goes smoothly.

Can I bring my other children to our appointment?

It is recommended that parents make childcare arrangements for siblings so that the parent and providers can focus on the child who was referred for an evaluation.
